- Technical or Customer-Facing Responsibilities: Utilizing healthcare information systems, electronic health records, and communication platforms to document patient interactions, track care plans, and engage with patients to ensure their care needs are met.
Required Skills and Qualifications
- Technical Skills: Proficiency in healthcare software systems, knowledge of medical coding and billing processes, familiarity with electronic health records (EHR), ability to use telehealth platforms for virtual patient interactions.
- Educational Requirements: Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or related field. Certification in Case Management (CCM) preferred.
- Experience Level: Minimum of 2-3 years of experience in care coordination, case management, or healthcare administration. Experience in a hospital or managed care setting is advantageous.

- Proficiency in a second language to support diverse patient populations and enhance communication in multicultural healthcare settings.
